Do you think both women were using the killing curse? The book does say “Fighting to kill”, but I'd assume that Molly Weasley wouldn't use Avada Kedavra. What spell was Molly using, then?
My best guess would be that Mrs. Weasley was using Stupefy.

In Half-Blood Prince, it said that multiple stunning spells were able to land McGonagall in a bed at St. Mungo's, so a powerful stunning spell straight to the heart (Where Molly landed the spell) would probably be enough kill.
(I haven't seen the movies yet, by the way.)
We know Bellatrix is a sadist so she probably wouldn't go straight for Avada Kedavra, but plenty of spells will kill you if you cast them enough times (Sectumsempra could probably do some lethal damage, and even a well-used Wingardium Leviosa would kill). I agree that Molly probably wouldn't use Avada Kedavra, but she probably has plenty of other lethal options, just less direct ones.
That's a good point. When looking at it like that, it seems likely that Bellatrix would even throw a few crucio curses in there too.
JK Rowling has said it was an extremely powerful stunning spell that hit her above the heart and caused a heart attack.
If she were fighting to kill, I doubt she'd be using Stupefy on the off-chance of stopping Bellatrix's heart. McGonagall was in danger because she was much much older than Bellatrix, and she took four at once. This doesn't mean that a single Stupefy to the chest of a middle-aged woman would come even close to killing her. I think someone fighting to kill would use Dark magic.
Why wouldn't Molly use the Avada Kedavra? because it's an Unforgivable or a very dark curse? Harry used the cruciatus and imperius curses, and while he wasn't prepared to use the killing curse we've seen that others from the Order were willing to use it
Plus it does matter on the state the caster is in. Remember Harry having problems causing the unforgivables until he was really emotional and wanted to hurt someone?

I’m not sure if Molly would use a curse like that even in that situation. I doubt she’d want to risk even the slightest chance of hitting an innocent bystander.
In the big group duels, many of the baddies use a range of spells, not just the killing curse. Not sure if this was alluded to in the Dept of Mystery battle or if I’m just inferring this with no backup but…I think the Inforgivables take longer to cast or require more sustained focus. You can fire off a lot more other spells (stupefy, taranticula, etc) than the more powerful ones. So it’s a tradeoff between speed and power
This going to sound really nerdy but I kinda imagine it being like some Mmo's pvp. It's a mix between dmg, control and out smarting your opponent. If you just castes Avada it become predictable, plus if you miss you could possible kill a friend.
Could have been a lot of things, the Killing Curse does nothing but kill, and does it instantly and unblockably. But lots of other spells can kill people. Like how Wormtail blew up a city block and killed a few dozen muggles when he framed Sirius
